International certificate of origin

The Pennsylvania Chamber of Business and Industry provides processing of International Certificates of Origin for businesses shipping goods to countries that require this documentation.

To certify documentation for a company, the following items will need to be completed and returned to the PA Chamber office (see Contact Information below):

Sample Letter template (PDF) - edited as needed to describe requested action
Certificate of Origin form (PDF) - must be received as a typed document
Help Index (PDF) - aids in completion of Certificate of Origin
Original signed invoice or equivalent - illustrating currency and value of items being shipped
Check made out to the PA Chamber of Business and Industry in the requested amount (see below)
Any other documents which provide proof of production in the US

FEES:

Members - first (5) free of charge; remaining $10 each
Non-members - $20.00 each
